You need an ID card or Welfare ID card with picture, birth certificate, S.S. card or green card, proof of a 2 year living history and reason for leaving your last address( like eviction letter, notarized letter from person you lived with.. Must have all people whom lived with you names, address and how long you was living with them.) And any medical records you may have on each person entering with you..
