Go to your family doctor and have him sign a notarized statement stating you have been known to him as his patient for _ years. Go to your highschool, votech or college and get transcripts and statements from teachers. Write down every job you ever had and when. Take all this info to the SS officr. Get a card. Order a copy of your birth certificate. If anyone gives you any garbage, go to legal aid for assistance. There is a formal process to help you regain your idenity. BEWARE if you are caught lying you will go to prison.

You'll need to go to vital records ave 1st get your birth certificate or talk to them about it. Then go to social security to get your ss card.
