They legally can not ask you to have a paper filed out by your Dr. that's a violation of your HIPPA, the ADA, and the Fair Housing Act. Just to let me know.
Good Morning Staci, For an ESA animal we have a form that we ask you to take to your doctor office to have them fill it out. The online registration paperwork is not something we accept as proof of a need for an ESA animal. If you would like the paperwork, just call us in the office and we can get it for you.
