What identification sources are needed to replace my social security card?

You need 3 forms of id. Ok medical doc's work. Birthcerticate, and just something w/Your name and address on it. Then go up to the office, it takes about 2 hrs.

There is a website that outlines the different accepted forms of i.d. but the forms i used are medical records and birth certificate.
