I have lost my birth certificate and need one for my real ID application. How do I get one through the Health Department? I was born in Burge Hospital (now Cox North). I was the City of Springfield Elevator Inspector for about 20 years.

You need to go in and fill out a form, then show your ID, if it matches your birth certificate name they will print you one right then.
