Had to replace my lost birth certificate. Very kind to deal with over the phone as well as in person! Lovely ladies working the front very attentive to everyone's needs. Thank you so much!

I want to know why they’re making my kids quarantine for 10 days when the CDC says only 5 days?!!!
Shari Alcorn
Why are they only giving out masks on the Southside of town? It would be a burden for Northside people to drive that far just to get masks. Surely the Library Station could also hand out masks.

Took only 15 min friendly and helpfull

can I get a shingles shot at Green County health department?
They will not replace the card but the health department will give you a copy of the immunizations
https://www.springfieldmo.gov/2890/Birth-Death-Certificates This has the information you need
"The Vital Records Office can provide certified copies of Missouri birth certificates for $15.00 each with a cash, check, credit card (authorized signature required), or money order payment. Birth certificates are available for most births of 1920 to present. The birth certificate copy will be printed while you wait. Vital Records Springfield-Greene County Health Department 227 East Chestnut Expressway Springfield, MO 65802"
Vaccines are given by appointment Monday thru Friday.
If you was born in Missouri you should have no problem
Great question! You need to contact the Missouri Department of Health's Bureau of Vital Records for assistance. You can call them at 573-751-6387.
The Health Department does not offer dental services, but they will refer you to Jordan Valley.
If you were born in Missouri otherwise they can answer questions
Idk I want there
It must be pretty safe, as I see people sleeping on the ground around there
That is a negative. Federal privacy laws won't allow that, in addition being up-to-date is better than going off of records, so you or the party in question depending on what ever situation are in. Should probably get tested at a local health clinic.
You need to go in and fill out a form, then show your ID, if it matches your birth certificate name they will print you one right then.
Hi Chris--here's the details: https://www.springfieldmo.gov/2890/Birth-Death-Certificates
Yes they have different kinds of test you can buy or I'm pretty sure the health department will test it for you if you take it up there
You need to contact the vital records office in Illinois to get a copy.
The clinic opens at 8am and closes when it is full. This can be very early some days so we always recommend you come as early as possible.
Our hours at the Bensch Bldg are varied. Vital Records is 8:00-4:30 STD clinic 8:00-3:00 Immunization clinic is by appointment only. Other departments of the Health Dept in other buildings may have different hours. Call 864-1683 if we can be of any help.
We can certainly help point you in the right direction! Our friends over at the Jordan Valley Community Health Center would be a great place to start your search for medical care. You can reach them at 417-831-0150.
Yes, you can come in and we will give you a printout.
I did contact Corporate and they said nothing they can do. I understand a service dog but no in the cart where your food and child go. Also doesn't the dog have to have service dog label on the dog
Sorry, we can only access certificates for people born in Missouri. They will need to contact the vital statistics department of their birth state.
Have you called Jordan Valley Clinic? The health department opens Monday at 8. If you call then, they may have a suggestion.
You may want to ask the people over at Building and Development.
Take a water sample to most any pool supply specialty store and they can test it for you for free or at minimal cost.
Goodluck with that but call these peeps they maybe able to help you with that
