You have to request it and they also sell the pineapple for an additional five or six dollars. They will sing and they will give you a hat and take a picture. It's worth the extra couple dollars.

Based on our last birthday at Tsukasa, they will do it, but not for free. The manager came over to us and discreetly asked if we wanted the birthday package for a fee. When we said no, there was no singing or birthday dessert.
