You call the number and leave a message. I leave on the message my name and number how many are in my household that are in need, and if you need toiletries, or baby items. And they will call you back but you have to answer their call when they do or you'll be put back on the waiting list. It could take up to a month before they can return your call. They will set you up with a date and time for pick up. As for requirements your first time they have a form to fill out with who all that you are requesting help for their birthdays I think last 4 of their social number and your phone number and address. Youll have to fill out the same form yearly but its simple.
