If you require a rental via your insurance provider, you should contact them directly. They will reserve your car using their own corporate account and in your name, attached to your insurance policy or claim number. Any standard fees are covered, within the expenditure of your policy. What is not covered are additional items such as Satellite Radio, child seat, etc. Unless you're referring to their inhouse insurance. That's a flat daily rate, that is added to your rental agreement, they can explain better if and how to file a claim directly with them, via their Customer Service Number.
