You must apply in your local DFCS office, take your ID, birth certificate, and proof of income. If working and paid weekly you will need four recent consecutive pay stubs if paid bi-weekly will need two recent consecutive pay stubs. Proof of rent & electric or gas bill. Acceptable verification is lease agreement or statement from Landlord.
Everything yu had the first time plus proof of residence
