If you are requesting records from VBA (service medical records/service treatment records), you can send a FOIA (Freedom of Information Act) request. All information and forms are located at https://www.va.gov/FOIA/Requests.asp . Click on the VBA link provided. If you are requesting records from VHA (the VA Hospital), you can go to the same website, click on VHA link to find your VA hospital and information. Hope this helps.
