Call the Moss Clinic tell him your condition they will get you in for registration they take a couple days to call back or you can call and ask for Miss Ruby
When i had the Experience,1.go an get an appointment with the irs ,for 2016 & 2017 taxies,. 2.and all of your passed doctor notes of your health.i think that's it??
Call set up appointment with Miss Ruby and she help you .But it good they have this clince but my opinion I dont recommend it cause of my feeling of some of my expirations .
Just go there and ask for an application.
