I sent in the wrong form. I've been advised to make an appointment at the Passport agency, and fill out a different form, bringing alon photos, and another form of ID. Should I just show up? Unable to reach anyone.

If you are in need of emergency service, I would contact your Senator and ask for assistance. Providing you have all your documents necessary, you could obtain an appointment very quickly.

Chicago doesn't take walk ins so you'll have to keep trying for the appointment. Join the Facebook group Passport Appointments (don't buy one, they're a scam) and they can help explain the whole process.
