Anywhere you try to take this dog they will need the consent of an adult to do anything medically. You can call and report it to animal control or the humane society and I don't believe they will try to verify your age since you are just reporting it.

Best advice: call and ask them. They can give you exact requirements, needs, etc. I've taken many strays in to turn over to the county. They've taken every one without question beyond where I found the dogs. The last one we took was so thin they didn't even wait to do paperwork. They took him directly from my hands and rushed him back for immediate attention. When I checked up on him a couple of days later he was healthy, happy, and being adopted. They do ask for ID, but I think they're just taking it to check to make sure the dogs I'm turning in aren't one of my own that I'm trying to pass off as a stray.
Call m the humane society and explain it to them. They will help you and the dog.
