Check your discharge papers. They normally refer to your family doctor for follow up unless you don't have one but you shouldn't be going back to the ER

You should first look at the discharge papers. If you do not have them, calling CHOP, they may be able to look up the records. For future reference having myCHOP might be good as you can look up records on the app.

There should be an appointment already made. They should have handed you a piece of paper or told you to follow up with your primary. If you didn't get one call to find out.
