My insurance covers my visits 100%. You will have to just aske when you get there, every insurance plan is different.

I go through my health insurance and pay about $60/visit. Weekly. I got my first visit free as a referral from a friend, so not sure about the start up fees. I've been a very happy client for five years now. Highly recommend them.
