Your best Avenue is to go to their web site and apply on line. After their review, they will notify you with an appointment to complete the application process and what documents you need to bring.
I did it all on line and on the phone. Never needed to go anywhere or provide any documents. Don't over think it.
Above is correct you can find all the information you need however if you don't know exactly what your looking for or where to navigate from there you just wasted your time. I myself have to say I'm not a big fan of computers, not a big fan of libraries or paying to print out information. If you are impatient like me or second guess yourself with some of the questions they ask. I am both & prefer to go in person. If you get there early enough ( I am so not a morning person) I try to get there before noon. I say this because the 1st time visit is normally the worst. Also keep in mind that without an appointment it can be hit or miss depending on how crowded it is. If you are applying for medical only they are going to want a copy of your income, a minimum of a month. They will also ask for more then 1 type of ID. Your Driver's License or the ID cards that DMV offer are above all one of the most important. Then a Birth Certificate, a bank account card (Debit). I always cover my butt.....
Eventually u will need financial statements ie bank, job etc so dtart to get them together
