Sarah, all of our patients were notified via letter on January 30, 2018 to whatever address we had for them on file at the time of closure about our March 2, 2018 closure date. (We did have some returned as bad addresses or people had moved, etc. And we did have some people tell us they didn't realize it was a letter verses a statement.) We also notified patients via our web page, Facebook page, extensive Facebook advertising to advise patients of the closure and in the newspaper. We have made arrangements for medical records to be distributed both to patients or requesting PCP offices through Total Care MedSuites, which is operating in the space that Liberty Family Physicians was in. There is a receptionist available at that location to download your medical record to a CD and give to you or mail to your home, or new provider. He is the most wonderful, kind, caring, supportive doctor! Such a terrific physician, undoubtedly an asset to this area!!
