You take your tag from the last time and they will switch it over to your new address and then you are set to get a tag for that area. Yes, and as long as your I D has your new address you're fine.
You will probably need a proof of address like lease, utility bill etc. I'd say check the info in the county tag office's website for the specifics. It should be an easy process, though.
