From the Dr. Office or the radiology office you went and had your procedure.
Call the hospital and ask for records department. I had done it.
You can call the hospital directly and ask them what department sends medical records.
I check mine online. If you go to their website click on my chart, fill out the info and you will have access to your mr. Don't know of that helps
