Why tell the manager? Report it to the health inspector. The health inspector sees this and will close that McDonald's until the problem is fixed.

Call the and ask for a manager. Or Google their corporate office.
Call the restaurant and ask for one. Although sounds pretty odd as I have never seen any roaches there. Maybe it was at your house and crawled into the bag.
There's also a number that you could call to report it on the receipt
Ask to speak to the manager .but i have eaten there probly 100x and i have never had any problems with roaches or foreign objects
