I am diabetic, and I am taking a longish international trip for which I will need to take insulin which requires refrigeration. Can I take a small medical pack with me with the insulin and an ice pack?

Yes. Get medical letter and there are special small ice cool pack for carry-on flight

Yes, they should allow you to take it because it's consider to be medication
