At Family Planning Associates you will need an ID with your face and name. If you are over 18 you will have legal paperwork to fill out stating that. If you are under 18 a parent/guardian is not required to come with you. The website has information for under 18 women, including judicial bypass. A PARENT WILL NOT BE CONTACTED WITH JUDICAL BYPASS OR WHEN THE NOTIFICATION FORM IS BROUGHT IN. The website has all the info. If you're coming in for gynecological services a parent is not notified either, no matter the age.
Maybe, if it's a college ID. But if you're under 18, a parent, or relative will be notified. It's a requirement if you're under 18.... depending on what you're there for.
