Sue, are you wanting a form to fill out prior to your vaccination? If so, you can stop by the office and pick up a form or you can go to maconmohealth.org and print a form off of our website.
You can pick up the form at the health department or we will have them at your clinic when you receive your second shot.
The Health Depart may have access to that for you, but I talked to my doctor first.
