you have to go online to print out the forms to make an appointment and form for the death certificate. fax it to the your local vital records and wait for a phone call to make an appointment. if you're from Philadelphia there is a vital records on 8th and arch next to penndot and the forms are outside for u to take but you still have to fax it over. no walk in avaliable just appointments.
