How do I go about obtaining a birth certificate with both parents name on it? My certificate does not have their name on it. My application was rejected for a passport because of this. Thanking you in advance.Jacqueline Midili

You have to Physically go inside a vital statistic office and fill out the paper application with both parents name listed on the application and they will print you out one for $20. If you are a veteran they will give you your birth certificate for free but you will have to show them proof with with your military ID. They will also issue to the military spouse for free as well. Idk if you have to ask for it that way. When I applied for mines it just had my name on it. When I replaced my kids it had both parent names.
