No, there is not a dress code. I have worn tie dye t-shirts to the rock shows, but I have also dressed up, at least dress casual level, for other performances. It is a very nice, but rather laid back environment.

There's not a set dress code for any show I've been to. But people usually want to look nice, so I would stay clear of cargo shorts or flip flops. :)

Not the 2 event I went to recently.. but there might be for some events.. not sure

It depends on the event, but generally no. I went to a show recently and 90% of the crowd was dressed street casual.
