what if the name on my card/account is different from my real name, back in the late 60's or early 70's when i got the card I used my nickname on card.... didn't think too much about it but now I'm getting closer to retirement age I need it fixed.

Yes you will have to take your Birth Certificate. Probably take 4 to 6 weeks to get it back, I think.

I'm sure you can just go in and fill out a form to change that. I'd take my birth certificate with me to prove it is you .
