I got a replacement card with just my driver's license, so it seems that a government issue photo ID is sufficient to replace a Social Security card. To get a new number, you presumably also need proof of citizenship or legal residency such as a birth certificate or green card and if you're getting a new number due to ID theft, you may need to provide evidence of harm such as a police report. I don't know what you'd need if applying for benefits (which is a different office down the hall, but at a minimum you should probably bring that government issue photo ID.
