Birth certificate Or Papers from your primary doctor with your birthday, name and information stating that you attend that hospital. It cant just be papers for an appointment. From what i know those arent allowed. An I.D Or you can use an insurance card but it vant just have your name. It needs other information. Also make sure its your real name. i made the mistake of using documents with only one of my last names (I have two) and unfortunately they refused to give me a social security card because of it. They are very strict on that. A
