What all do I need to send in to get my name changed

But I don't have a ride all the way up there but I have had them loose my stuff before this will be the second time but thanks guess I'm gonna have to try to get up there I wonder if I could fax or email them my documents

As of April 1, 2022 they are open to the public without an appointment. First come first serve. I would go in so you do not have to worry about them losing your documents! Good luck.
