Only issue is the site says they dont take medicaid card for proof of identity for child but the do apparently . and the things they said they do take as proof the refused! They also don't print copies of the number anymore. I dont have transportation so it was a little frustarating going back and forth

You cannot do online. Either you have to appear in person at your nearest SSA office or you can obtain a form, either print off of the website or get one from your library. If you mail it in you MUST send in with the application your picture ID such as drivers license or state ID. You can also call your nearest office to get instructions.
