You need two forms of ID (drivers license and birth certificate) you go in there there's paperwork to fill out and you wait for your number to be called and go up to the counter and you a paper one then and 7th in 10 business days you will get your new Social Security card. You can also go on to socialsecurity.gov website and get all the information there as well
I have them my ssn and answered a few questions and they ordered me a new one
DL, State ID, US passport. No paper or temp IDs.
