Never had to appeal! Have all of your Paperwork in order chronologically. Have doctor's diagnosis, scheduled and previous presedures paperwork!
Their legal aid might be able to help. I've never needed the help that way.
If you can not find help,call 311 the mayor office,see if that help
From my knowledge appeals are pretty much a denial. Long as you have the correct paperwork you have no problems.
All they provide is how you can file the appeal and other resources and people that you can contact
