Don't know if you know it or not but you have to have an appointment first then all i used was my current ID and the money order they ask you to wait to fill it out. I just gave the lady my blank money order she filled it out and I was done within 35 mins try to get an 8:30 am appointment they move fast in the morning
You need your social security card and if you have it your ID and a 25 dollar money order then fill out the form there afterwards they will have you wait 30mins to an hour and then they will ask you a question about your parents to verify your the right person then you should get your birth certificate
