I had an appointment with a doctor who is licenced through the state to prescribe Cannabis for individuals with qualifying conditions. The list of those conditions can be found on the Department of Health website. After you have a referral letter then you apply for a permit through the Department of Health. The PA that I saw is an amazing guy. Well educated and kind. His name was Tim Pickett and he's the founder of Utah Marijuana.org.
