Your will need to take a disabled person application to your doctor's office. You can find one online at: https://www.tn.gov/content/dam/tn/revenue/documents/forms/titlereg/f1310301Fill-in.pdf Please make sure section E on page 2 is completely filled out by your doctor. Please bring the completed form to the County Clerk's office to be issued a placard, license plate, or decal.
