Usually, a bad leak. Running though with radiator cap off you should be able to see water flow in the radiator. DO NOT ATTEMPT TO REMOVE CAP IF HOT! Be safe.
Like peter said, if it leaks. You can sometimes remove the radiator cap (cold), warm up the engine and see if you get fluid flow.
If it leaks generally.
