How don't report medicaid card lost or stolen?

I lost my Medicaid card. I need a replacement card, to bring it to the doctors office. How do I get another card? I do know my account number on the card. Who do I contact? I don't have my card to call the number on the back of the card. How can I reach out to DSS? I need this card for my doctors office.

All you have to do is call the number in the back of your food stamp card follow the steps until it says report a lost or stolen card and it's very easy after that just follow the steps
