When in doubt, fast. Any test that measures the level of a substance/medicine/supplement usually requires fasting. It may also require the person to stop taking the substance/medicine for a specific period of time before the test, as well. In some cases, you could be instructed to stop taking other substances/medicines/supplements as well because they can also interfere with the accuracy/reliability of the test results. Because of this, would highly recommend that you verify directly with your doctor and/or the lab for instructions specific to your particular case, especially since there may be other factors specific to your case that only they would be able to properly instruct you on (like recommending the test be taken at a certain time of the day or month, etc.). Best of hopes!
