No. But it is a nice restaurant, so while no dress code per se, I wouldn't show up in cut-offs and t-shirt. But jeans and a nice top are perfectly fine.

You would be ok with a little less than bus casual, but definitely if your going to wear jeans have a jacket and collared shirt, no stated rule though
I would say yes, although there is no posted dress code, and the atmosphere is relaxed.
