I have done special celebrations. You talk with Disney Dining people and actually reserve a Celebration Package with them. Of course that costs more but they have some really nice packages. If you do not go that route, when you make your reservations, you put in that you are celebrating a Birthday and for which member of your family. At Crystal Palace,Towards the end of your meal, Cast members will bring you a card signed by all the Characters there and a Birthday cupcake presentation. We have done it several times that way at that restaurant. For that, there is no extra cost. It is nice and of course singing Happy Birthday.
I would suggest going directly over to the hostess as soon as you get in the park. This will allow you to get a good idea of what can be done and give them time to accomodate before your dinner reservation.

I know they have a very nice birthday cake for sale it's $35 and definitely enough to feed 6 the staff did a really good job at singing Happy Birthday also
