For some strange reason my insurance was canceled as of march of this year. I didn't get an notification in the mail or e-mail saying why. How do i go about re applying

Same thing they did to me cancel my insurance in a few months back never got anything in the mail was still paying on my bill monthly had no promble taking my money I'm going for all these test for my heart did not know I had no health insurance now I'm stuck will $$$$$$ in debt with hospital bills thanks to them
