Service dogs are allowed everywhere under federal law... See 28 CFR 35.136 - Service animals. The only time, according to federal law, that a service animal may be asked to leave is if the animal is not house broken or if it is out of control.
Yes legally they have to allow service animals. Beware, I’m currently training a medical alert service dog and we have had many times we have almost been attacked! Not joking one dog was off leash and came at my dog barking and growling, I kicked the dog away from my dog and complained. Half the dogs in there wearing vests are NOT SERVICE ANIMALS!
Yes. They actually allow any well behaved dogs.

I have taken my dogs at most north county malls, and never had an issue as long as they behave. Including non service pets.
All Westfield malls allow dogs... on a leash and well behaved
