I would recommend writing kind, detailed, and unbiased feedback through whatever channel they prefer, every time it's an issue. It really shouldn't be on us, the customers, to alert them to issues, but, as someone who actually works in food service, I appreciate the detailed feedback because it helps me troubleshoot. Or, just don't go. I'm sorry you're experiencing problems regularly!
Because it's only since the last couple of months; my guess would be that it is someone new that is messing up your orders. So, mentioning it (nicely) is the feedback the managers need in order to review the new person's peformance.
