Make an appointment for a cédula because you can't get the passport without it. You can get the passport in the same appointment but it must be an appointment for a cédula, not an appointment for a passport. Bring your birth certificate and your US passport to the appointment. You will need three pictures for the cédula (which can be taken next door, the photographer there is familiar with the exact dimensions). The passport picture is taken during the appointment by consulate staff. The process is tedious, complicated, and confusing but it is the only process available in the US. Good luck.
