The doctor is there Tuesdays and Thursdays for evaluations. You don't get the card while there but you can get many questions answered.you will have to complete an online application at the dph website to get the card.
The Dennis location does walk ins Tuesday's and Thursdays. Bring your 200 bucks MA identification and your medical records for which your getting your card and when you get approved you will get your paper work with a number to call and register your info. Within 10 days you will recieve your card. Happy pain free smoking.
They have very strange hours I do not believe they are open on Mondays
