You can request a replacement Alliance Member ID card on our website at www.ccah-alliance.org/aspnetforms/MemberOrderForm.aspx. You can also call Alliance Member Services at 800-700-3874 (TTY 800-735-2929 or 711). It can take up to 10 business days to receive your new ID card.
