Florida casual is all you need.
Not really, but I wouldn't want to be overly casual. No sleeveless shirts, but jeans, shorts, or flip flops are all probably okay.

No. Shirt and shorts are just fine.
No, but last time I went, I was dressed in my work dress shirt and pants. I regretted that decision as eating lobster can get very messy. Wear something that you can just throw in the wash afterwards. Jeans and a nice cotton shirt should be fine.
