If you go through the entire assessment process, a report complete with diagnosis(es) and signed by a licensed clinician. I don't know about a specific form. I know the evaluation itself is a two-step process. The first part is done by a trained, qualified intake specialist/therapist, and if in their clinical and professional opinion you require additional services, you then move to be seen by the staff physician. The initial part of the assessment is free of cost, last I knew, but seeing the physician is where the assessment fee of $60 comes into play. They assess for all substances but only provide treatment for select types of drugs - opioid treatment, if I'm not mistaken. There are other agencies in the area that will provide treatment for any conditions found that are not managed by JTC. The federal government and Base accepts their reports; I suspect courts would too.
I cant say for certain ,but they(counselors)go above and beyond what is required of them- to make sure that you are getting the right treatment, being treated right, and taking advantage of every possibility and form of treatment available to you.
